Terms and Conditions for X-on Health Ltd (“X-on Health”) Customer Feedback prize draw

  1. The Prize Draw is free to enter. You must be a resident of the UK and be aged 18 or over. The prize draw is only open to partners and employees of X-on Health customers who received the relevant Customer Survey.
  2. The Prize Draw is not open to employees of X-on Health, their families or anyone else directly or indirectly involved in the running or promotion of the Prize Draw. X-on Health reserves the right in its sole discretion to exclude any person from the Prize Draw at any time.
  3. X-on Health will handle the administration of the feedback exercise and Prize Draw entries.
  4. To enter the prize draw, please click on the link to complete the feedback exercise. If you have any questions about this prize draw, you can contact us at orders@x-on.co.uk, putting ‘Customer Feedback Prize Draw entry’ in the title of the email.
  5. Only one entry is permitted per person using the link sent. The opening date for entries will be 12th August 2025 at 9 am. Closing date for entries is 12th September 2025 at 5 pm, although X-on Health reserves the right to extend this to increase the number of responses to the survey. 
  6. There is one prize of £1,000 value to be won. X-on Health will draw the winner in a computer-generated random draw.
  7. Notification of a winning entry: The winner will be notified by X-on Health by email and confirmed by a telephone call that they have won, within 14 days of the prize draw. Notification will be sent to the email address to which the feedback exercise link was sent. A winner must respond within 14 days of the notification that they have won a prize to confirm whether they would like to make a donation of £1,000 to their nominated charity or to receive goods and services from X-on Health to the value of £1,000 (inclusive of any VAT applicable).  Any charity nominated must be registered appropriately with the Charities Commission. 
  8. If the winner does not respond within 14 days, they will forfeit their prize, and X-on Health reserves the right to randomly choose an alternative winner. The winner will receive their prize within 28 days of the prize draw.
  9. All decisions as to the winners of the Prize Draw will be at X-on Health’s sole discretion and will be final and binding. 
  10. Details of the winners will be available to participants upon written request and the names of prize winners will be publicised in the X-on Health customer newsletter and/or other communication channels, including social media.
  11. These Rules are governed by the laws of England and Wales.
  12. For the purposes of the research conducted as part of the prize draw (including an entrant’s survey responses), X-on Health is the data controller, and the external agency (Savanta) is the processor.
